To receive coverage under the plan, you are required to enroll in these programs prior to receiving services:

- Fertility Solutions
- Transplant Resource Services
Voluntary Clinical Programs
These programs are available to you, but are not required to receive coverage under the plan:
- Cancer Management Program
- Congenital Heart Disease Resource Services
- Maternity Support Program
- Neonatal Resource Services
