Choice Plus (CPN)
If you are unable to locate a network provider within a 30-mile radius of your home ZIP code, call UnitedHealthcare to see if you qualify for a gap exception to use a non-network provider at the network benefit level. When there is not a network provider within a 50-mile radius of your home ZIP code and you must travel at least 50 miles to receive care from a network provider, the plan provides a travel and lodging allowance for all covered health services provided by a network provider. Transition of Care
If you’re pregnant or receiving certain services – like select cancer therapies, end-stage renal dialysis or a transplant – and your provider isn’t in the network, you temporarily may be able to continue with that provider at the network benefit level. Download and complete the UnitedHealthcare Transition of Care form below and send it to UnitedHealthcare to see if you qualify for Transition of Care.
